AI models are rapidly proliferating, but fragmented channels for reporting harmful incidents and technical defects often leave people unsure where to raise concerns. FLARE-AI, an open-source platform led by a group of AI researchers, aims to provide a single portal for recording and tracking cases while improving transparency and safety accountability among developers and research institutions.

FLARE-AI recently formally launched and began accepting public reports of harm and defects caused by AI models. It forwards cases to the relevant developers and research institutions for action.
