AI models are rapidly proliferating, but fragmented channels for reporting harmful incidents and technical defects often leave people unsure where to raise concerns. FLARE-AI, an open-source platform led by a group of AI researchers, aims to provide a single portal for recording and tracking cases while improving transparency and safety accountability among developers and research institutions.
FLARE-AI recently formally launched and began accepting public reports of harm and defects caused by AI models. It forwards cases to the relevant developers and research institutions for action. As of July 20, 2026, available information did not disclose the exact launch date, total number of reports, participating institutions or funding amount.
