Taiwan Health Ministry Builds National SMART on FHIR Infrastructure for Smart Healthcare Ecosystem

Taiwan’s Ministry of Health and Welfare has pursued its Next-Generation Digital Healthcare Platform Program since 2024. The initiative uses FHIR to standardize medical-record formats and SMART on FHIR to standardize app integration and authorization, addressing incompatibilities among more than 40 hospital information systems across Taiwan. Its central aim is to allow AI diagnostic and clinical tools to be deployed across hospitals, narrowing the digital divide between medical centers and small and midsize hospitals.

In its latest step, the ministry unveiled FHIR Box on May 20, 2026, after completing an interoperability demonstration at Linkou Chang Gung Memorial Hospital, Mackay Memorial Hospital and Chung Shan Medical University Hospital by the end of 2025. Deployment is planned for all medical centers in Taiwan by the end of 2026 and will expand to regional and district hospitals by the end of 2027. The inaugural “Taiwan 50” program also selected 50 apps, with each team receiving NT$100,000. The system processes more than 2,500 FHIR Resources per second.
